Consent to Access and Credit Repair Services
By signing below, you consent to ThriveBox LLC accessing, reviewing, and working on your credit report as part of our credit repair services. This agreement covers the setup and monthly fees associated with our credit improvement process, as well as credit monitoring services.

Services and Billing Structure

Setup Fee: $139.00
Billed after your initial credit report analysis and customized action plan.
Monthly Service Fee: $139.00
Billed monthly after documented results are achieved, such as successful disputes or updates to your credit report.
Credit Monitoring Fee: $1 trial for 7 days then $19.95/mo after
Essential for tracking progress. By signing, you authorize this charge.
Note: Credit monitoring services are provided by (Credit Hero Score). You may cancel anytime, but this may affect our ability to track your progress.

Consent to Obtain and Dispute Credit Report Information
By signing, you authorize ThriveBox LLC to:

Obtain and review your credit reports from Experian, Equifax, and TransUnion.
Dispute any inaccurate or unverifiable information on your credit report with credit bureaus or creditors.

No Guarantees
You acknowledge that ThriveBox LLC does not guarantee specific improvements to your credit score or removal of all disputed items. Results vary based on credit bureaus' and creditors' responses.

By signing below, you acknowledge that you have read, understood, and agree to the terms outlined in this consent and service agreement. You authorize ThriveBox LLC to charge the setup fee, monthly service fee, and credit monitoring fee as described above.
